Then either:
a.) Complete the Form, Print it and Sign it manually and Post. 

NOTE:  If you fill the form online it CANNOT be saved - only printed. (ctrl + P). Saving will only keep a blank copy.

b.)  Save form to your PC,  
     Complete and Sign digitally with Adobe Reader & Email.
  1. Right-click on form and select "save as". Choose where to save the document and click "save"
  2. Close the form in view and open the saved version with Adobe Reader.

  3. Complete the whole form leaving out the signature fields. 
  4. Digitally sign the form in the two signature placeholders. If this is the first time you have signed a file digitally you may need to set up your digital signature. When signing, Adobe Reader will ask you to save the file.
  5. To send to CCSM:
    Go to file > Send file > Attach to Email.  Add your ID photo as a second attachment before sending to     newzealand@amccsm.org
